To submit a proposal, please read the guidelines, then click on the button to download the form.  Once you have copied the form with your own information on it, save it and email the proposal form by clicking the "Email Your Proposal" button.  Make sure you attach the form and any relevant materials you wish to include.  A response email will be sent back within 24 hours to verify receipt of your proposal.  If you have not received a response within 24 hours, please resend the form through the email button. 

Please note whether your presentation proposal will provide continuing education credit.  Continuing education proposals must have this form completed to be considered.  It is desireable for all presentations to be eligible for continuing education units, and this will be considered in the final selection process.
